account number with California DMV, to access MVRs.
If your organization presently has an open account
with California DMV, just fax us a copy of your INF1146 form, to give
you immediate access to our California online MVR services. Fax number
is 305.253.1440.
No, I do not have an existing account with
California DMV, to access MVRs.
If you currently do not have an open account with
California DMV, please follow the enclosed instructions. This packet
contains the documents that the California Department of Motor Vehicles
(DMV) requires in order to obtain Driving Records and Vehicle
Registration Reports from the state of California.
IMPORTANT: The DMV requires
both the original application and a photocopy with original
signatures. Photocopies of signatures are not allowed.
To complete the applications:
- Complete all pages
- Photocopy all pages
- Sign ALL applications in blue
ink. (Both the originals and the photocopies)
Please forward your application to:
Department of Motor Vehicles, Accounts Processing, Unit-H221,
P.O. Box 944231, Sacramento, CA 94244-2310 and be sure to include:
- Completed and signed California
End User Agreement and California
Online End User Agreement.
- A check or money order for
$50.00 (for non-address verification record) payable to the DMV.
- A copy of your insurance license,
business license or permissible documentation.
Once the DMV approves your
application, you will receive a document containing your
company's: CA Agreement Number, DMV Requestor Codes and Expiration Date.
Upon receipt of this document, please fax a copy to our office as soon
as possible. The DMV does not forward this information to
our office. Please fax it over to our office at 305.253.1440.
