Prior to completing the Online Application
In
order to facilitate the process of completing
the online application, please make sure that
you have
the following information* readily available:
- Federal Tax ID
- Professional/Occupational License (if any)
- Driver License number (The driver’s license number provided will
serve as a unique identifier)
- Bank account information
- Errors & Omissions Coverage (if any)
- Credit Reference contact information (at least one)
Upon submission, a copy of the agreement and
selected state forms will be displayed on your
web browser which will be available to print.
Documents will also be automatically sent to you
in a confirmation email.

After Submitting the Online Application
In order to expedite the online application
assessment process, please provide us with the
required documentation and at least two of the
optional documents indicated in the table below.
Required Documents are based on industry type.
